Patient journey


Doctor’s office

Once you have made the decision in consultation with your doctor for your day surgery – the doctor’s reception staff will request an admission to Medical Forum Theatre (MFT). The doctors’ practice will inform you of your surgery date as well as your arrival time at MFT.

Your medical aid cover, co-payments for doctors’ fees as well as your fasting times (if necessary) will also be discussed in the doctor’s rooms.

Medical Forum Theatre: Pre-Admissions and Admissions

Your date for surgery will be set and you will be required to present your admission form to MFT at
least two (2) days prior to your surgery to “book your bed”.

When “booking your bed” prior to your surgery our staff will confirm your details and discuss the particulars of admission process, these may include Medical Aid cover, authorization number from and any other necessary documentation which will need to be completed.

Please ensure that you, the patient, phones your medical aid to obtain a pre authorization number. This number must be obtained before your day of surgery. Should there be a co-payment our accounts department will inform you telephonically thereof.

You may either visit MFT to hand deliver your admissions forms, download the forms and return them by email or fax or submit the forms online.

Please bring the following with you:

  • The Main Members ID book or membership certificate no older than 30 days
  • The Patients ID book (in the case of a minor, a copy of the birth certificate)
  • Medical Aid Card reflecting details of main member & all dependents
  • Proof of authorization received from your medical aid
  • Consent Form – signed by your attending doctor

Fasting

GENERAL ANAESTHETIC – patients may not eat, drink (even water), smoke or chew anything for at least 6 hours prior to your procedure. This may be longer depending on advice from your admitting doctor. Please ensure that you confirm your fasting period with his/her rooms.

LOCAL ANAESTHETIC - No fasting is required unless specified by your Doctor.
Should you be on medication please inform your admitting doctor/specialist.
Please consult your attending doctor for instruction.


Medical Forum Theatre: Surgery Day

On your surgery day please ensure that all your documentation that was required during the preadmissions was submitted. If not please take the needed documentation with.

Please leave the following items at home:

- Firearms and Weapons. All weapons are prohibited on the premises -
Medical Forum Theatre is a gun free building
- Jewellery or valuables

Your stay with us will be about 3 - 4 hours, please understand this time is just a guideline.
We suggest that you bring something with you to pass the time.

Arrival

On arrival at MFT please present yourself to our reception staff. The receptionists will confirm that all the necessary paperwork has been completed and that all your details are correct.

You will then be asked to take a seat in the waiting area until one of the pre-operative nurses calls you.

Pre-operative assessment

Our nursing staff will assist you with all preparations for your procedure – this may include checking your vitals, weight and record of any current prescription medication or pre-existing conditions.

Upon completion of your pre-operative assessment you will be led through to the ward where you will wait in our comfortable beds/recliners until your doctor is ready for you in theatre.

After the procedure

After your procedure you will be assisted in recovery where our friendly nurses will monitor your progress. Once you have fully recovered you may be offered a light refreshment.

Please ensure that you have arranged transport for yourself as you should not drive a motor vehicle for twenty four hours following general or local anaesthetic. Our nursing staff will contact your carer/partner/friend or relative to collect you.

Settling your account

If you have Medical Aid please confirm with them that you are covered for your procedure or if there
is an applicable co-payment.

If you are required to make a co-payment or are a private patient your fees will be payable in full
prior to surgery.

Please note that Medical Forum Theatre’s fee does do not include your doctor’s fee, the anaesthetist’s
fee or pathology.

You will be charged separately for:

- Medical Forum Theatre Fees (payable before your procedure)
- Anaesthetist
- Surgeon
- Pathology (if applicable)

You may pay by cheque (Bank Guaranteed Cheque), by card, by means of an EFT / Direct Deposit or cash. If you are paying by card please check your daily withdrawal limit as it may not be enough to cover the full amount payable.
